Youth Development Bush Adventure Programs
Single day and sequential 8 week programs

  • Please complete the form below to register a young person’s interest in our Youth Development Bush Adventure Programs. Programs may have a recreational purpose and be once-off such as holiday programs or be more enriching such as our sequential 8 week programs.
  • All information gathered will be handled in accordance with MYST’s Privacy Policy. Please note that we will only store referral information about young persons that end up participating in the program.
  • After receiving the information we’ll make contact to discuss if the program is able to provide your young person with the best possible experience. We’ll also chat about safety, and how we can ensure the program is safe and inclusive for everyone that attends.

Medical Administration Consent

I understand that:

  • Mountain Youth Services Team Inc are not authorised to administer any prescription medication that is not provided by the parent/guardian and is not accompanied by the appropriate instructions as presented on this form.
  • If your young person/ward is anaphylactic and requires an EpiPen, we require that two (2) valid EpiPens be sent along on the day. As per Mountain Youth Services Team Inc’s procedures, one EpiPen stays with your young person/ward at all times and the other is held by the facilitator.

I understand and agree that:

  • I will provide the correct amount of medication for my young person/ward for the duration of the activity.
  • I will ensure the medication is separated into individual dosage for each time it is required to be administered,
  • I will ensure the medication is appropriately packaged with the clear, original label, in a container, zip lock bag or blister pack, and comes with instructions.
  • I will ensure medication is provided for all relevant medical conditions pertaining to my young person/ward.
  • I will ensure that the medication provided is within it’s expiration date.
  • I will provide a copy of all relevant medical action plans, for example: Anaphylaxis & Epipens Action Plan and Asthma Action Plan.

  • The purpose of the study has been explained and I agree to participate in the study.
  • I realise I may withdraw consent at any time, without reason. This will not impact the service I receive.
  • I understand that participation is voluntary. I understand that participation in the study will include completing two short measures each session.
  • I understand that the information collected will be kept confidential, will only be used for the purposes of this research, and no identifiable information will be shared with the chief researcher. I have been advised as to what data is being collected, what the purpose is, and what will be with the data upon completion of the research.

I understand that the activities which are a part of Mountains Youth Services Teams Outdoor Explore Program may include Bushwalking (on and off track), Canoeing, Mountain-biking, Abseiling, Rock Climbing, Swimming, Camping, Yoga, Raft Building, Low/High Ropes Course and Flying Fox Riding

  • Abseiling is the activity of descending down a rope, usually in a cliff environment, and including heights.
  • Bushwalking is the activity of walking in the natural environment which means it can involve walking up and down steep terrain, uneven surfaces, over, under and around various hazards and walking along rock edges.
  • Rock Climbing is the ascent of cliff faces using ropes and two belayers as a safety backup, and involving heights.
  • Mountain Biking consists of riding bicycles off road, often over rough terrain, over, under and around various hazards.
  • Swimming includes swimming in inland waterways such as rivers, creeks, streams, lakes, dams and lagoons where, dependent on factors such as swimming ability, water clarity, depth and temperature, participants may be required to wear Personal Flotation Devices (PFD’s). Swimming may include water entry techniques, such as jumping (from heights no greater than 10 metres) or sliding into water.
